From 11/1/09 to 11/30/09, all benefit-eligible employees have the opportunity to add or increase their optional life and/or long-term disability insurance coverage.  Enrollment forms for both life and long-term disability insurance are available in your campus mailbox.  Enrollment forms must be completed, signed and returned to the Human Resources office by Nov. 30, 2009 whether or not you are changing or adding coverage....
